Rates

$175 per assessment/intake

$125 per 50 minute individual session

$145 per 50 minute couples/family session

Insurance

At this time, I am currently in network with Aetna, Cigna, United Healthcare/Optum, Anthem Blue Cross/Blue Shield of Ohio and Texas, ComPsych, Caresource, and Medical Mutual of Ohio. Depending on your current health insurance provider, it is possible for services to be covered in full or in part. A superbill can be provided after the session where a patient can submit it to get reimbursed for out of network benefits. I do offer a sliding scale fee if needed. Please contact your insurance provider to verify how your plan compensates you for psychotherapy services.

I would recommend you ask these questions when talking with your health insurance provider:

1. Does my health insurance plan include mental health benefits?
2. Do I have a deductible? If so, what is it and have I met it yet?
3. Do I need written approval from my primary care physician in order for services to be covered?

Payment

I require electronic payments before services are rendered. Payments/Co-payments are expected to be paid the day of your session unless payment arrangements have been made with me in advance. I can invoice you through my electronic health records system which is HIPAA secure.

Cancellation Policy

If you are unable to make a scheduled appointment, please contact me to cancel at least 48 hours in advance from your scheduled appointment. Otherwise, you may be charged for the full session rate.
